I liked the forms. It would have helped me to make better decisions when I was with Prudential. Thanks for taking action on this.
One suggestion is to have it be less like a form and more like a receipt. Stores are very successful at letting me know how much taxes were charged. Not that you have to change the form, just add a summary section with stuff listed out like a receipt. I think the information will be more digestable in that format.
