Katz: It is not clear to me how the notarized document will be drafted to confirm the authenticity of the Form ID?? Is the notarized document to be a separate document or can the notarization be stated on the Form ID itself? Can the Form ID contain the notary language near the signatory line of the applicant? I do not see the need for a separate document stating the authenticity of the Form ID. My suggestion would be to have the notary language stated on the Form ID. The language could be stated, simply, "Signed and acknowledged before me." Space must be allocated for the stamp of the notary as well as the signature line of the notary. Above or near the signature line of the applicant there should be a statement of authentication by the applicant, such as, I certify that the above information is true and correct, etc., etc. If I can be of further help, do not hesitate to contact me.
