Our chief operating decision maker reviews financial information presented on a consolidated basis for purposes of allocating resources and evaluating financial performance. There are no segment managers who are held accountable by the chief operating decision maker, or anyone else, for operations, operating results and planning for levels or components below the consolidated unit level. Accordingly, we determined we have a single operating segment. During the years ended December 31, 2015, 2014 and 2013, 94.3%, 94.7% and 95.8% of our revenue, respectively, and substantially all of our long-lived assets were attributable to operations in the United States.
