3. Shares held in the name of filer's daughter, Natalie Seiffert.
4. Shares held in the name of filer's daughter, Michelle Seiffert.
5. The options were granted 12/21/2012 and vest in five equal installments of 20 percent each year beginning one year after the grant of the options.
