3. Restricted Shares granted pursuant to the 2009 Restricted Stock Plan which vest on the second anniversary of the date of grant.
4. Restricted Shares granted pursuant to the 2009 Restricted Stock Plan which vest on the third anniversary of the date of grant.
5. Shares withheld by issuer to pay tax on vested stock previously granted under the 2006 Incentive Plan.
Remarks:
This Form 4 is being filed by Reporting Person to correct certain transactions incorrectly reported by the Reporting person from the time Reporting Person first became subject to the filing requirements of Section 16 of the Securities Exchange Act of 1934, as amended, in November 1992 (the "Initial Filing Date"). For ease of reference, this Form 4, collectively with the Form 4 filed on April 22, 2015 (Date of Earliest Transaction: 03/17/2011), includes all of Reporting Person's transactions in the securities of the Issuer since the Initial Filing Date.
